Junior Prep Stars Application. IMPORTANT NOTICE: If you have submitted an online application before Monday March 22, 2010, PLEASE reapply using this form. Unfortunately, we had configuration problems and the form results did not get to us before this date.

Fill out the application completely and submit it.
Then print out a copy of the insurance liability release form, sign it, and mail it in.
Mail your non-refundable deposit ($85). Any balance is due on the first day of camp. The easiest way is to mail both your deposit and insurance release form together, to this address:

Players may not participate without turning in a signed release form.
A player's spot in a camp or league is not guaranteed until the deposit and release form are received. Please note that we must receive BOTH a deposit and release form for EACH camp a child will attend.
